The Name Change report has room for twenty-five members on each page. Either printout a blank form and then fill it out, or load the form on your computer, fill it out, save it, and then print it out and mail it. Because no money (check) is involved with the Name Change form, it may also be emailed.
The members national number should be listed in the Member # block.
The name of the person the way it is shown on the roster, should be listed first.
The new first name and/or the new last name should be listed in the Names space.
