Filling out the Loss Report

The Loss report has room for twenty-five members on each page. Either printout a blank form and then fill it out, or load the form on your computer, fill it out, save it, and then print it out and mail it. Because no money (check) is involved with the loss form, it may also be emailed.

The full name should be listed in the Name space.

The members national number should be listed in the member # block.

The reason block refers to why the loss of member is ocurring. The choices are TO for Transfering Out, D for Deceased, R for Resigned, S for Suspended (Non Payment of Dues), and DUP for Duplicate (For removing a name listed twice on the National Roster).

A word of caution about deleting members. Once a member is deleted, they are gone. If they quit and then rejoin, they are assigned a new membership number. When someone resigns from your department, be sure they are also resigning from the lodge. Many lodges allow the retired/resigned members to continue membership. This is normally determined by the lodges Constitution and Bylaws.

Suspending members for non payment of percapita on a local basis is recommended rather than doing it on a national basis. It is quite frankly too hard to track. Wait until your bylaws provide that a member can be dropped, and then use the loss form to drop the member. (This is why the S catagory is listed on the form).
